Recovery from Hospital Treatment
After a long hospital stay or surgery procedure, patients may need a brief
period of adjustment or monitoring before returning home. In the transitional
care program, patients work with knowledgeable medical professionals who
evaluate the level of care they will need at home. This process can include
instructing the patient on valuable exercises, diet concerns, medication
routines, and providing home caregivers with directions on how to facilitate
the patient’s care.
What to Expect
Patients recovering from all manner of health concerns utilize this facility.
In order to ensure everyone is accommodated, we staff a multidisciplinary
care team of nurses, physicians, physical therapists, medical specialties,
and a support staff. The ultimate goal of this program is to help patients
readjust to home life. In that effort, our staff pays personal attention
to your goals and desires so they can customize treatment according to
your specific situation.
During the transitional care treatment, you may be moved to different levels
of care as the severity of your condition deescalates. Gradually, a patient’s
condition should improve to the point where they are ready to return home.
