Due to COVID-19 Patient Financial Services and the Medical Records office
is closed to walk-in traffic. Please

If you wish to obtain a copy of your medical records, you must sign a release
of information form and bring it to the medical records department at
Titusville Area Hospital. For your security, medical records staff will
ask to see your photo ID and ask for your date of birth to verify your
identity. If you are not able to bring the form to TAH, you can designate
another individual to pick up your medical records for you. If this is
the case, you must indicate on the form the individual that is picking
up your medical records. The individual must also present photo ID and
you will be contacted via phone to verify that the individual is permitted
to take your records. The formal request for the release of your healthcare
information/medical records is a safe guard to protect your health care
information mandated by the Health Information Portability and Accountability
