Give us your name, telephone number, program name and the date you wish to attend.  Make checks payable to Titusville Area Hospital and are due at the first class unless other arrangements are made.

CPR and FIRST AID (two-session class)
Dates in 2008:   August 25 & 27; October 27 & November 3; December 2 & 4
Time:  6:30 p.m. to 9:30 p.m.
Place:  Titusville Area Hospital
Fee:  $30

CPR Recertification - Healthcare Provider (single sessions)
Dates in 2008:   August 21st; September 18th; October 16th; November 20th
Time: September and November classes held from 1to 3 p.m.; August class held 5 to 7 p.m.; October class held from 3 to 5 p.m. 
Place:  Titusville Area Hospital
Fee: $30

DIABETES MANAGEMENT (4-session class)
Dates in 2008:  September 9, 11, 16 & 18 from 7 to 8:30 p.m.
Place:  Titusville Area Hospital
Fee:  $10

Details:  This educational program will be presented by Judy Paulich, R.N. who will cover medications, preventing complications, exercise, monitoring blood sugar and a discussion on how to better cope with disease.  Carolyn Bienio, Registered Dietician, will also present information on menu planning, restaurant dining and food exchange lists. The program is very helpful with handling the day to day issues of living with diabetes.

MEDICAL TERMINOLOGY (Beginner)(8-session class)

NUTRITION - "PERSONAL NUTRITION COACHING"
Dates in 2008: By appointment only.
Place:  Titusville Area Hospital
Fee:  Free to Highmark members, $50 initial visit and $25 follow-up visits for non-Highmark members

Details:  You an registered dietician, Carolyn Bienio, RD, design a personalized eating plan related to heart health, diabetes or weight management.   

OSTEOPOROSIS (Prevention and Management) "HOPE" (6-session class)

Time: 4 to 6 p.m.
Place:  Titusville Area Hospital
Fee:  Free to Highmark members, $90 for non-Highmark members

Details:  Focus on exercise, nutrition and stress management to build and maintain healthy bones while learning to make healthy lifestyle choices.   

STRESS MANAGEMENT - "DISCOVER RELAXATION WITHIN"  (4-session class)
Dates in 2008:  September 3, 10, 17 & 24 or November 5, 12, 19, & 26
Time: 5 to 7 pm

Fee:  Free to Highmark members, $50 for non-Highmark members

Details:  A lifestyle improvement program to identify and reduce individual stressors through learned relaxation techniques. 

TOBACCO QUIT PROGRAMS -SEE THREE PROGRAM DESCRIPTIONS BELOW:

TOBACCO "QUIT" Program - "CLEAR THE AIR" (single session)
Dates in 2008: August 28th or December 4th
Time: 5:00 p.m. to 7:00 p.m.
Place:  Titusville Area Hospital
Fee:  Free for Highmark members, $35 for non-Highmark members

Details:  This one-session course is designed to help you quit smoking. 

TOBACCO  "Quit" Program  - "FREEDOM FROM SMOKING" (8-SESSIONS)
Dates in 2008:   October 9th through November 20th (8-sessions)
Time:  5:00 -7:00 pm
Place:  Titusville Area Hospital
Fee:  Free

Details:  A tobacco cessation program for adults using a group behavior modification technique. Education topics and group interaction teach participants how to quit using tobacco. Call for a brochure of the program's outline.  FREE nicotine patches, gum and lozenges, Chantix or Zyban if you are uninsured or your insurance does not cover these quit aides.

CHEW TOBACCO  "Quit" Program - "ENOUGH SNUFF" (Free In-home program)
Free In-home Program to help you quit chew tobacco.  Helps you break the habit by
1) getting ready to quit, 2) planning to quit, 3) dealing with withdrawal, 4) staying off chew or snuff. 
Receive Free:  do-it-yourself book, video with quit tips, and phone counseling
You Must be:  18 years of age or older, be a daily user of chew tobacco for one year or more, and willing to quit all tobacco use including snuff, chew, cigarettes, cigars and pipes.
